There are 4 documents that must be returned as part of your 2025-2026 registration. The documents are linked below. Please thoroughly read each document. You will need to:

- sign,
- AND upload the completed documents here. Make sure to orient your documents vertically before uploading them. The required documents are:
- Statement of Faith, Financial Contract, Waiver/Liability Release, and Student Conduct and Behavior Form
NOTE: If you have multiple children, please scan the Student Conduct pages and upload them in 1 document.
